Many laboratory managers judge the success of their safety program by looking at one number—the injury rate. If no one has been injured this month, things must be going well, right? Or are they?
The reality is that injury rates tell us only what has already happened. They are lagging indicators, meaning they report yesterday's problems rather than tomorrow's risks. By the time an employee experiences a needlestick, chemical splash, or back injury, the opportunity to prevent that event has already passed.
The strongest laboratory safety programs don't wait for injuries before taking action. They use data to identify weaknesses long before someone gets hurt. That requires looking beyond OSHA recordables and tracking a broader picture of laboratory safety.
Near misses reveal risks before injuries occur
One of the most valuable indicators in the laboratory is near-miss reporting. A near miss represents an accident that almost happened but fortunately did not result in injury. Many organizations may overlook these events because "nothing happened." In reality, these reports often provide the best opportunity for prevention and improvement.
Imagine a specimen leaking during transport but never exposing an employee, or a staff member nearly slipping on a wet floor before regaining balance. These events reveal weaknesses in work processes that deserve attention just as much as actual injuries. Encouraging employees to report near misses also builds transparency, which is a hallmark of a healthy safety culture.
A low number of near-miss reports isn’t automatically good news. It may indicate excellent hazard control, but it could also signal that employees are reluctant to report mistakes or do not recognize the near- misses when they occur. Compare near-miss reporting with other safety indicators like safety observations, employee survey results, and audit findings. If all results are positive, low reporting likely reflects a strong culture. If audits uncover issues but near-miss reporting does not, there may be a reporting culture issue at play.
The quality of near-miss reports matters as much as the quantity. Reports should include enough information to understand what happened, why it happened, and what changes can prevent a repeat occurrence. When employees see their reports leading to meaningful improvements, reporting naturally increases.
Track whether corrective actions lead to lasting change
Corrective actions are another important metric. Many laboratories identify safety issues during inspections or incident investigations, yet some of those corrective actions remain open for weeks or months. An unresolved safety issue will remain a hazard. Lab safety professionals should track how quickly corrective actions are completed. More importantly, they should review overdue items regularly. If corrective actions continually miss deadlines, ask why. Is there inadequate leadership support? Are resources unavailable? Has ownership not been assigned? Measuring closure times helps identify bottlenecks before they become larger problems.
Closing a corrective action should mean more than checking a box. Safety leaders should verify that the action eliminated the root cause, that the change was implemented as intended, and that it did not introduce a new workflow or safety concern. A brief follow-up observation or audit can confirm that the hazard has truly been controlled.
Repeat safety findings deserve special attention as well. If an inspection repeatedly identifies improperly labeled chemicals, blocked eyewash stations, or employees failing to wear eye protection, the problem probably isn't the individual employee. It may indicate ineffective education, poor supervision, unclear expectations, or a workplace culture that has accepted unsafe practices as normal. Repeated findings should never be viewed as separate incidents. Instead, they signal an opportunity to investigate the underlying process.
Personal protective equipment compliance is another observation metric that can provide valuable insight. Many laboratories perform routine safety rounds, but few trend PPE compliance over time. Track observations involving laboratory coats, eye and face protection, gloves, and other required equipment. Look for patterns rather than isolated violations. Do compliance rates improve over time? Does one shift consistently perform better than another? Do violations increase in specific departments or around certain procedures? These trends often identify opportunities for coaching rather than discipline.
Measure participation, not just policies
Participation in the safety program by laboratory leaders and staff can also reveal the strength of a safety culture. Track attendance at safety committee meetings, completion of departmental safety rounds, or participation in voluntary safety initiatives. While these may seem like simple administrative numbers, they are actually powerful leading indicators. Organizations where leaders and employees actively participate in safety discussions typically identify hazards sooner and resolve them more effectively. A safety committee that frequently cancels meetings or struggles to maintain attendance sends an unintended message that safety is no longer a priority.
Lab safety training metrics also deserve close examination. Too often, laboratories measure training completion rather than learning. Completing an online module doesn't necessarily mean employees understand the material. Instead, evaluate training effectiveness through competency assessments, drills, observation, or simple conversations during safety rounds. Ask employees how they would respond to a chemical spill or where to locate a Safety Data Sheet. These discussions provide far more meaningful information than a signed attendance sheet. Consider tracking first-time competency success rates or the percentage of employees requiring retraining after annual competency assessments. These metrics can reveal weaknesses in training effectiveness before they contribute to workplace incidents.
Use audits and minor incidents to uncover hidden risk
Safety audits remain a very strong predictor of laboratory safety—but only if they're performed honestly. Research has shown that organizations conducting frequent, meaningful safety audits generally experience better safety outcomes. Interestingly, organizations demanding perfect audit scores often develop poorer safety cultures because employees become reluctant to document problems. Conversely, laboratories reporting excessive deficiencies without correcting them also struggle.
The healthiest audit programs typically identify a moderate number of issues and resolve them promptly. Honest auditing followed by timely action builds credibility with employees and demonstrates that reporting hazards leads to positive change rather than criticism.
Finally, don't overlook incident trends in the lab that fall below OSHA reporting requirements. Paper cuts, minor bumps, small chemical splashes, and other seemingly insignificant events may appear unimportant individually. Collectively, however, they often reveal patterns that larger injuries simply haven't exposed yet. Laboratories should be encouraged to report every injury, regardless of severity. Minor incidents can turn into bigger health issues, and reporting them can help identify trends, improve training, and strengthen the overall safety program. More importantly, this philosophy promotes openness rather than discouraging employees from speaking up. A transparent reporting culture gives laboratory leaders far more information with which to make good decisions.
Ultimately, measuring safety isn't about creating more spreadsheets. It's about understanding what the numbers are trying to tell you. If your only safety metric is the injury rate, you're looking in the rearview mirror. But when you monitor near misses, corrective actions, repeat findings, PPE observations, audit results, committee participation, and training effectiveness, you begin to see where your safety culture is headed—not just where it has been.
