Quality assurance reviews for lab testing are systematic processes embedded into laboratory operations that ensure the test results produced are accurate, reliable, and meet regulatory standards. Involving procedural checks, equipment functioning, documentation, and data integrity, laboratorians can identify and correct errors to achieve consistent, high-quality laboratory performance

Patient care, for example, depends on the quality of the clinical laboratory testing that informs it. Successful quality reviews are accomplished through a focus on consistency, correctness, coherence, clarity, conformance, and concordance. By adopting these six “Cs,” lab managers can lead their facilities to higher quality standards, benefiting both the lab and the population it serves.

Six Cs for success

The first “C” is consistency and involves using reasonable and consistent units of data and formulas, as well as ensuring that appropriate figures are utilized in the results reported. Consistency is critical to the quality review process—only through the use of consistent units, formulas, and figures can accuracy, reliability, and data reproducibility in lab testing results be achieved. 

By standardizing procedures in the laboratory, variability can be minimized, which additionally assists in error detection and regulatory compliance maintenance. Quality reviews conducted on a regular basis allow for the identification of deviations from procedures that might compromise patient safety. Establishing consistency means that every staff member will interpret lab results in the same fashion. It builds a foundation of trust in the results and, for diagnostic labs, can enhance patient care through diagnostic precision.

Correctness is the second “C.” In successful quality assurance reviews, achieving correctness means that all measurements are within identified working ranges and each instrument was calibrated using recognized standards. Reviews reveal whether the lab test result calculations can be verified, beginning with the raw data input and including all remaining aspects, including up to the summary provided in the result’s report. For clinical labs, errors in reporting test results can lead to incorrect diagnoses that may lead to inappropriate patient interventions or missed diagnoses. Adhering to established protocols ensures correctness in lab data and promotes confidence in the test results. Without accurate results, lab tests cannot be trusted as reliable. 

The third “C,” coherence, mandates that all assumptions be clearly stated and each conclusion aligns with the data produced. Quality reviews will uncover whether the conclusions determined in the lab report follow the data presented. If there are any inconsistencies between the data and conclusions drawn, these should be clearly stated and discussed in the lab report. Discrepancies should be flagged and explained. Coherent reporting addresses communication breakdown between the lab and its consumers, reducing misunderstandings in data interpretation. This helps to support decision-making based on the lab test results through clear and logical data presentation, enhancing result reliability.

Clarity comprises the fourth “C” and ensures that jargon and unclear terms are defined such that the final lab report is digestible for the reader. Were there any acronyms utilized? If so, these should be clearly defined and explained such that a person with limited technical education could clearly understand and put the information to use. In healthcare, clarity is the only way that lab data can be effectively utilized by clinicians. Quality lab reports present data by defining the units used alongside their reference ranges and interpretations, which minimizes misuse of lab data. Quality assurance reviews uncover ambiguous and/or confusing terms to enhance report readability and foster trust in the laboratory services provided, as the results are reported transparently, precisely, and in an actionable manner for the data’s end users.

The fifth “C,” conformance, involves utilizing audits and validations to check that the results produced deliver on the intentions of the testing provided. Was the quality assurance plan followed? Are there appropriate calibrations and other quality control checks employed to validate the lab data? During the quality review of conformance, staff should verify that lab test results align with established standards and compliance requirements. Test methods, documentation, and reporting should follow institutional and regulatory guidelines. Conformance efforts help achieve uniformity, reduce errors, and maintain compliance. Adhering to conformance procedures promotes laboratory credibility and valid test results.

Concordance is the sixth and final “C.” Its objectives are precision, accuracy, representativeness, comparability, and completeness. Concordance helps laboratories achieve alignment between their findings, previous results, and expectations. If the lab data’s quality indicator goals were achieved in these areas, the lab has solid concordance. Quality assurance reviews for concordance can help identify discrepancies that lead to errors or test limitations. Concordance fosters consistency, the first “C,” and it enhances collaboration between laboratories and their clients, ensuring test results are coherent and relevant.

Effective quality assurance reviews are essential to producing meaningful lab test results. The six Cs provide a structured, comprehensive framework for evaluating and improving laboratory practices. Each “C” plays a vital role in reducing errors, supporting accurate lab results, and ensuring regulatory compliance through identifying discrepancies and streamlining data review procedures. An investment in continuous improvement through the deployment of quality assurance measures means that lab integrity is maintained and trust in laboratory services is thereby achieved. 

When applied together, the six Cs strengthen data integrity, enable laboratorians to have confidence in the reliability of results they report, streamline lab efficiency, and build confidence in laboratory operations. A commitment to these six pillars of quality excellence is where high-quality lab testing begins.
